Grooming Release Form

PROOF OF RABIES VACCINE IS REQUIRED FOR GROOMING.

If proof of Rabies is not provided, then your pet will receive a Rabies vaccine. This will be done at the owner’s expense. We will use a flea and/or tick product if fleas or ticks are seen on your pet during grooming. This is done at the owner’s expense. (Approximately $5.00 – $20.00)

If your pet's coat is too matted to clip as discussed, Town & Country Animal Hospital has permission to cut the hair shorter if necessary. Close clips may run a risk of razor burn.
